IMPORTANT  INFORMATION REGARDING

CHANGES TO COVID-19 MEDICATION DELIVERY SERVICE

 

On the 1st August 2020 the Government guidelines regarding shielded and vulnerable patients are being relaxed. This means that our extended delivery  service will stop from this date.

If you are currently using our extended service you will need to make other arrangements for collection. Alternatively, you can contact the Community Hub Service on 01482 393919 for assistance with collection and delivery.

To be eligible to continue to use our free Medication Home Delivery service you must fall into the  housebound category and have no other means of collecting your medication. Please get in touch with your dispensary if you feel this applies to  you.

Please note that deliveries to our community drop-off locations will continue as normal.

Patient Services Manager (Administration & Estates)

Part Time 30 Hours per week

We have an amazing administrative team waiting for the right leader to join them and we know you will learn as much from them as they do from you.  You will report directly to the Head of Operations so will have a genuine opportunity to shape our services for the benefit of our patients.

Responsible for the day to day management of our administrative, secretarial and correspondence management staff, you will ensure that all our administrative functions run smoothly and efficiently.  Other key functions include the management of our buildings and equipment, health and safety compliance, and information governance. 

We are looking for a dynamic, professional individual who is great working under pressure both as part of a large team and individually.  You will be able to prioritise your workload and be an excellent communicator, able to negotiate effectively and to smooth ruffled feathers when circumstances require it.  You must have experience of managing staff, providing leadership and support, and a proven ability to design and implement effective systems and processes.

We operate from 7 locations across Holderness.  Your week will be mainly split between our sites in Hedon and Withernsea where most of your team members are based but there will be regular travel between all our sites.  You will need a car for work and a flexible approach to hot desking, recognising that plans may need to adapt at short notice to respond to changing priorities.

Pharmacy Technician

Part Time-Full Time

We are looking for a self-motivated and organised Pharmacy Technician to join our friendly multi-disciplinary team. Applicants must be registered with the General Practice Pharmaceutical Council and should hold a full driving licence.

Overview of Role

To assist in the delivery of the Holderness Health Prescribing and Medicines Management Plan and the delivery of patient services as determined by Holderness Health policy and local and national guidance.

Main duties of the job

To follow agreed protocols, referring to Clinical Pharmacists or GP's as appropriate. Follow local and national Infection Control guidelines. Contribute to achievement of the highest possible quality standards such as the QOF and other agreed Key Performance Indicator targets. Attend in-house educational and staff meetings as appropriate. Assist with the training of other staff members and also with the education of doctors and nurses in training as appropriate. Positively facilitate change. Demonstrate pro-active engagement with the practice population and wider community to promote healthy living and encourage uptake of services. This may include visits to local schools, community centres and other groups as necessary.

We are the largest GP practice in the East Riding and are seeking an Advanced Clinical Practitioner to join our multi-disciplinary team on either a full time or part-time basis.

We have the scale, diversity and breadth of skills within our large team to help you to develop your career, pursue areas of interest and take advantage of the opportunities we offer.  With truly local control we intend to keep the best of traditional patient centred Primary Care with all the benefits of a larger organisation.  Most importantly for you, we have extensive experience in providing a supportive environment for ACP’S as part of our diverse clinical team.  

About this Role

To provide care for the presenting patient and demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice. Diagnose and treat patients who present with minor ailments and minor injuries, supported by GPs, advanced nurse practitioners and other members of the nursing team. Attend urgent home visits. The advanced clinical practitioner will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures. They will have the ability to prioritise and make any necessary referrals for investigations in the appropriate manner and be flexible and responsive to patient needs.

About You

You will have a relevant qualification at Degree Level or equivalent as well as having up to date membership of a relevant professional register.

Post qualification experience is essential to this role.

We are recruiting for a Clinical Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists within general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

About You

You will have a relevant qualification at Masters Level or equivalent as well as having professional registration with the General Pharmaceutical Council.

A minimum of 2 years post qualification experience is essential to this role.
