Senior Clinical Pharmacist

Full-time, 37.5 hours per week

£42,414 - £49,969 dependent on experience

 

Are you looking for an opportunity to use your clinical pharmacy skills to the full, helping to shape patient services within general practice in the East Riding? We are looking for an experienced pharmacist to join our multi-disciplinary team in a patient-facing role.

 

We are Holderness Health – a brand new organisation which came together on 1st October 2018 from our three founding practices Church View Surgery, Hedon Group Practice and South Holderness Medical Practice. We are now the largest practice in the East Riding of Yorkshire and have significant ambitions for the future.

 

This varied role will include:

 

  • Responsibility for areas of chronic disease management
  • Clinical medication reviews to proactively manage patients with complex polypharmacy
  • Providing support to general practice staff with regards to prescription and medication queries
  • Supporting the repeat prescriptions system
  • Dealing with acute prescription requests
  • Medicines reconciliation on transfer of care
  • Implementing systems for safer prescribing
  • Providing clinical leadership on medicines optimisation and quality improvement
  • Managing aspects of the QOF framework and enhanced services

You will have a minimum of 5 years post-qualification experience and will already be an independent prescriber or working towards achieving your prescribing qualification.

We are Holderness Health, a new organisation formed from the merger of our three founding general practices and now the largest practice in the East Riding of Yorkshire. We are busy building our new and exciting future and our Chief Executive Officer is looking for a Personal Assistant to join her team – someone who can fill a pivotal role in making sure that everything runs smoothly and efficiently.

 

The role will include all the traditional PA activities but will also allow you to challenge yourself by taking on a variety of projects and leading on our internal and external communications. Every day will be busy, your workload will be varied and we will need you to feel confident showing initiative.

 

You will need to be able to demonstrate substantial experience of diary and inbox management, minute taking and office management. You will also need fast, accurate typing skills and advanced knowledge of Microsoft Office.

Note Summariser

Full-time, 6 month temporary contract

 

We are looking to appoint a temporary Note Summariser.

 

Main duties include:

  • Sorting patient records in preparation for summarising
  • Reviewing medical records and producing an accurate summary of the patient’s medical history
  • Read coding the information into the clinical system
  • Monitoring progress of notes summarising against practice targets
  • Filing and retrieving paperwork
  • Actioning GP2GP incoming records following a protocol
  • Other general administrative duties
  • Willing to travel as required for the role, including working from any of our 7 sites (access to a car for work is therefore essential)

General Practitioners will be encouraged to develop/enhance specialist interests whether that is clinical, teaching or management, but if an applicant wishes to spend their time in direct patient care, we also welcome this addition to our team.
