View your Medical Record Online
The Hedon Group Practice offer patients the opportunity to view detailed coded information of their medical records online using the system Patient Access, other providers are available if you wish to use them (Evergreen life and Dimec).
If you would like to view your medical record, you will need to read our Information Leaflet (FAQ), and request a consent form at Reception. Please bring photo ID with you when submitting your consent form (e.g. a passport, driving licence).
Your medical record will be available for online viewing no later than 28 days from your request.

After activation, if you see someone else’s medical information you should immediately exit from the system and inform the practice staff.
If there is any missing information (e.g. immunisations given elsewhere) or there are any incorrect details, please complete an amendment form and we will review and amend if appropriate. To complete an amendment form, you can either print the form off below or request it from reception.

*If you are already registered for Patient Access online, simply read the Information Leaflet, and complete the Consent Form and return it to the practice.
**If you are not already registered for Patient Access please read about how to register by clicking here, you may register for online appointment booking at the same time you bring in your registration form.
***Please allow up to 28 days for us to process the approval for access to your medical record. Once you have been activated you will be contacted by reception.
