Practice Bank Holiday closures in December

The Practice will be closed on Monday 25 and Tuesday 26 December 2017, the Practice will re-open (usual time) on Wednesday 27 December 2017.

If you require urgent medical advice during the closure please contact our out of hour service on 111.

View your Medical Records Online

The Hedon Group Practice offer patients the opportunity to view areas of their medical records online using the system Patient Access.

If you would like to view your medical record, you will need to read our Information Leaflet (FAQ), and request a consent form at Reception. Please bring photo ID with you when submitting your consent form (e.g. a passport, driving licence).

Your medical record will be available for online viewing no later than 28 days from your request.

Friends and Family Test (FFT)

The FFT is a feedback tool which offers patients of NHS-funded services the opportunity to provide feedback about the care and treatment they have received.

The FFT tool combines a simple question, asking patients how likely they would be to recommend the service they have received to their friends and family; with at least one complementary follow-up question to enable patients to provide further detail about their experience.

To provide your feedback, please click on the "Friends and Family Test" link on the right navigation pane under the "Have your say" section OR ask at reception for a form.

On Line Appointment Booking

We have a range of appointments that can be booked (or cancelled) on line. Please complete the necessary documentation available from reception.

To reduce the number of missed appointments we are now sending patients a text message containing their appointment details when a booking is made.

A further message will also be sent 48 hours before the appointment is due as a reminder.

Facebook

Please note: We do not have an official facebook page. This is something we are looking at developing in the future. The Hedon Group Practice (HGP) facebook pages which are currently on the internet, were not set up and are not managed by HGP. Anything on the unofficial facebook pages are not monitored by the Management or GPs at HGP.

If you would like to view your medical record, you will need to read our Information Leaflet (FAQ), and request a consent form at Reception. Please bring photo ID with you when submitting your consent form (e.g. a passport, driving licence).

Your medical record will be available for online viewing no later than 28 days from your request.

After activation, if you see someone else’s medical information you should immediately exit from the system and inform the practice staff.

If there is any missing information (e.g. immunisations given elsewhere) or there are any incorrect details, please complete an amendment form and we will review and amend if appropriate. To complete an amendment form, you can either print the form off below or request it from reception.

*If you are already registered for Patient Access online, simply read the Information Leaflet, and complete the Consent Form and return it to the practice.

**If you are not already registered for Patient Access please read about how to register by clicking here, you may register for online appointment booking at the same time you bring in your registration form.

***Please allow up to 28 days for us to process the approval for access to your medical record. Once you have been activated you will be contacted by reception.
